Good syndrome (GS) refers to adult-onset immunodeficiency syndrome (B and T cell immunodeficiency) associated with a thymoma. We present a case of GS involving a 67-year-old female who had oral thrush for 8 years, recurrent sinopulmonary infections for 2 years, and a history of surgery for an anterior mediastinal mass that proved to be a benign thymoma. During the current admission, she suffered from another bout of sinopulmonary infection, and a decreased serum immunoglobulin level was noted on routine chemistry. The IgG, IgA, and IgM immunoglobulin subtypes were decreased markedly. With the diagnosis of GS, she was treated with intravenous immunoglobulin (IVIG), and showed clinical improvement. However, the sinopulmonary infection recurred after 9 months. On re-administering IVIG along with adequate antibiotics, her condition resolved again. (Korean J Med 75:S935-S941, 2008)
